Round face and gaunt face

How to correct a too round or chubby face.

The problem of an excessively round face is a common problem for many. First of all, it is necessary to evaluate the morphology of the tissues to understand the real structure: is it excess fat or lack of bone salinity? The next step is to modify some elements of the face on the computer, looking for those that are most capable of positively impacting the shape.

How to correct adipose tissue and bone salience

1) The adipose tissue can be aspirated with a very careful microliposuction or removed in its component of the cheek (bichat blister).
The blister: the removal of Bichat’s adipose blister (Buccal Fat Pad Removal) is an operation indicated for those who already have a well-defined face, not excessively fatty and wish to create greater depression in correspondence with the so-called dimples. It is not indicated for excessively round faces because the benefit obtained from it is poor.

2) Bone salinity can be increased temporarily (e.g. hyaluronic acid), definitively (osteointegrating prostheses) or it may be necessary to resort to lipostructure.

Causes of gaunt face

An excessively gaunt face may be due to:
– constitution
– excessive weight loss
– administration of particular drugs that cause the reabsorption of adipose tissue (e.g. antiretroviral drugs).

An excessively thin face is then exasperated by the presence of imposing bone prominence (cheekbones, chin, jaw).

Treatments to correct a gaunt face

The various interventions for the correction of a gaunt face must aim to restore volume to the cheek by implanting volumetric substances. The types of interventions that surgery offers to solve the problem of a gaunt face are:

Lipostructure (transplantation of your own fat)

Lipostructure is a method that aims to increase tissue volume by injection of fat taken from the patient himself. “Lipostructure” is the term coined by the American Sydney Colemann to describe the technique which involves the removal of adipose tissue by means of a very small suction cannula, its centrifugation and reinjection of the same with a refined method that allows achieving results of far superior in quality and durability to traditional lipofilling. The lipostructure method allows for maximum engraftment of the transplanted adipose cells.
The lipostructure operation can take place under pure local anesthesia or with sedation or under general anesthesia depending on the extent and site to be treated. The post-treatment period is characterized by marked edema which decreases in the following days.
The question everyone asks is: “How long does the injected adipose tissue last?” The scientific community has different opinions regarding the duration and percentage of rooting, in fact there are those who speak of 30% of taking root, some even of 80%. According to the “fat guru” Sydney Colemann, what remains at 3 months is definitive and generally what remains at 3 months is around 40-50% of the volume injected initially and can be considered stable over time. In some cases it is therefore advisable to repeat the procedure 6 months later. Unfortunately, it is not possible to inject significant quantities of adipose tissue because this would not take root and would reduce the possibility of neovascularization.

Filler

All absorbable and non-absorbable fillers are potentially capable of creating a volumetric increase but over the years only one has proven to be truly effective and with a very low complication rate: polylactic acid (commercially known as Newfill and today Sculptra). It is a substance capable of stimulating collagenogenesis, the process in which fibroblasts are involved. In summary, it is injected and stimulates the formation of natural collagen in large quantities. The process is widely scalable. Two-three sessions are required, rarely for greater fillings are 4 sessions spaced at least a month apart. Through these injections truly amazing results can be achieved in a volumetric sense.
