info@massimilianobrambilla.itLipostructure is an advanced adipose grafting technique aimed at volumetrically increasing tissues while simultaneously enhancing their trophic condition. It is an evolution of lipofilling that allows the implantation of a cell population characterized by maximum engraftment capabilities.

The fat is harvested under local anesthesia using a thin cannula with a tip designed specifically not to damage the fat. The microcannula is connected to a syringe handled by the operator to control the negative suction pressure, obtaining fat that is still vital and ideal for transplantation. The fat in the syringe is allowed to settle and then centrifuged, discarding the oil obtained from the breakdown of dead cells and excess fluids (anesthetic, water, blood). The injection is performed with very small syringes (1 ml) connected to an extremely thin cannula (1.5 mm). The injected fat is distributed at all levels in small tunnels to create a structural enhancement of the area to be augmented and promote maximum engraftment.
With traditional lipofilling techniques, the fat is not centrifuged, and the deposition technique involves fewer steps and a non-structural distribution in the receiving tissues.
The major challenge in fat transplantation (which is very delicate) is its survival. With lipostructure, the fat is manipulated with great care and deposited in individual particles in the receiving tissues (muscle, cartilage, skin, fat, etc.). This layering creates a large contact surface between the tissues, ensuring optimal nourishment of the transplanted fat particles. This is crucial to allow the fat to survive in the first few days before new blood vessels form to permanently supply it.

Indications
- volumetric increase of the cheekbones;
- improvement of under-eye hollows;
- filling of nasolabial folds;
- volumetric increase of the lips;
- treatment of facial atrophies and tissue depletion;
- treatment of aging hands;
- volumetric and selective breast augmentation;
- volumetric and selective buttock augmentation;
- correction of outcomes from liposuction.
Liposculpture refers to a form of body shaping that goes beyond simple liposuction. It is a procedure that considers the body in three dimensions and aims to shape its volumes while respecting overall harmony. Sometimes, the body to be shaped presents excess fat as well as indentations, fatty bulges, and a lack of projection in anatomical parts. For example, even in the presence of excess lateral fat, the buttocks may appear sagging and lacking projection. What to do in this case? If one does not wish to resort to buttock implantation (a procedure burdened with a high rate of complications), the condition can be improved by treating it with lipofilling: part of the fat tissue taken from the lateral parts is appropriately aspirated, processed, filtered, and re-injected into the buttocks to increase their projection. Lipofilling in the liposculpture phase is also very useful in the outcomes of liposuction, where depressions caused by improper use of the cannula are evident.
