The skin is composed of various elements, including the epidermis and the dermis: the former serves a protective function for the body, while the latter provides support to the epidermis and is responsible for the firmness of the skin. The compact structure of the dermis is due to the presence of collagen fibers and elastic fibers, the loss of which is one of the main factors in aging.

The combined action of internal and external factors leads to the appearance of wrinkles.
Internal factors are related to the normal aging process of our tissues and to conditions of illness, which progressively lead to the hardening (keratinization) of the epidermis, relaxation of the dermis, hypotonia of “mimic” muscle bands, and decreased hydration.
Over time, the cell turnover of the epidermis becomes more difficult because the outermost layer (stratum corneum) becomes drier and thicker, hindering the live cells; at the same time, the dermis and the deep tissues of the face (including facial bands and muscles) relax, causing a loss of firmness and the progressive deepening of wrinkles. Additionally, the gradual loss of efficiency in the cutaneous microcirculation prevents perfect blood supply to the outermost layers of the skin.
External agents capable of negatively affecting the skin, such as excessive sun exposure, heat, chemical agents, pollution, etc., accelerate the aging process.
To slow down the skin aging process, it is advisable to drink plenty of water and massage the most marked areas with moisturizing creams to increase tone and elasticity. Nutrition is also important, especially the intake of fats. If wrinkles are already in an advanced stage, the most effective results are achieved through aesthetic medicine and plastic surgery.
There are two types of specific lasers available: CO2 lasers and Erbium lasers. The most innovative lasers are the so-called “fractional lasers,” which combine greater maneuverability with increased safety and significantly faster recovery compared to traditional lasers.
Both types of lasers allow for the removal of damaged skin layers, smoothing wrinkles and scars, and eliminating skin blemishes.

Before the treatment, it is necessary to prepare the skin with specific creams. On the day before the treatment, to prevent possible infections, anti-herpes pills and antibiotics must be taken.
The CO2 laser acts by vaporizing tissues and is ideal in cases of highly damaged and less elastic skin. It not only removes the most superficial layers but is also capable of causing a significant contraction of the collagen and elastic fibers in the dermis, making the skin much firmer (“lifting effect”). It has been demonstrated that a single treatment stimulates abundant collagen production for years. Another advantage of the laser is the precision in leveling the treated areas to the desired thickness, leaving nothing to chance. On the downside, the post-operative period is quite demanding, and redness persists for a long time (about 3 months).

The Erbium laser, on the other hand, is much gentler than CO2; it works by causing micro-explosions (photoablation) of the superficial skin cells, and unlike CO2, it does not overheat the deeper layers of the dermis. This results in a much faster postoperative period, limited redness over time, but also slightly inferior results. It is therefore mainly indicated for refreshing the face and eliminating superficial wrinkles. Once healed, the appearance of the skin is significantly altered: new, fresh, pink, delicate, and velvety skin; wrinkles are greatly reduced, often disappeared, the skin texture is soft, and the consistency significantly increased. If you are planning to undergo laser resurfacing, plan for 5/7 days away from your activities (you will be too red to go out), and only from the 5th day will it be possible to apply makeup.
If the wrinkles to be eliminated are localized, local anesthesia is sufficient (lasting about 20/40 minutes). If the entire face is to be treated, it is preferable to use deep sedation (40 minutes of intervention in day hospital).
