The terms Lipostructure
The term “lipostruttura” was coined by the American Sydney Coleman to describe the technique involving the harvesting of adipose tissue using a very small suction cannula, its centrifugation, and reinjection using a refined method that allows for far superior results in terms of quality and duration compared to traditional lipofilling. The technique used in lipostruttura is designed to achieve the maximum engraftment of transplanted adipose cells, although other techniques for harvesting and transplanting have also been described, yielding good results. The fame of Coleman’s technique is primarily attributed to the brilliant idea and the fact that Coleman was the first to conduct scientifically relevant studies on the subject.
Lipostructure surgery
Lipostructure can be performed under pure local anesthesia, with sedation, or under general anesthesia, depending on the extent and location to be treated. Adipose tissue is harvested, centrifuged, and then reinjected. The post-treatment period is characterized by significant swelling, which gradually decreases in the following days. The injection sites can vary widely: the face with reduced tone that needs firmness, depressed areas after trauma, the buttocks during liposuction, and even the vagina. Lipostruttura is also very useful in cases of damage from radiotherapy because it can induce tissue regeneration.

Duration and Engraftment of Transplanted Adipose Tissue
The question everyone asks is: “But how long does the injected adipose tissue last?” Worldwide literature disagrees on the duration and engraftment percentage, with some citing 30% and others 80%. According to the “fat guru” Sydney Coleman, what remains at 3 months is definitive, usually about 40-50% of the initially injected volume. In some cases, it is advisable to repeat the procedure after 6 months. Unfortunately, it is not possible to inject significant amounts of adipose tissue because it would not engraft and would reduce the chances of neovascularization.
